Our Model of Care

The HealthActions Approach

We believe that movement, free from pain, is essential for optimal health and well-being. We have utilized our extensive Physical Therapy experience to create a FOUR PHASE system of movement management.

1

Calm The System

The goal of PHASE 1 is to focus on calming your movement system.  This is accomplished by treatment of symptoms through movement- based or manual methods.  At this level, you are at the lowest level of function and often speak of acute pain and muscle spasms.

Treatment at this point centers on reducing tissue and joint loading along with strain. Your team will also work to provide education on your condition while they strive to identify positions and movements that reduce symptoms and move your care into Phase 2.

2

Restore Movement to Your Muscles & Joints

The goal of this phase is to restore much needed motor control in an effort to move closer to optimal functional movement.  For you to move into PHASE 2 the original chief complaint must be resolved, or easily controlled, through the application of techniques learned during Phase 1.

At this point you would have an understanding of your symptoms as well as what triggers your pain. In Phase 2 joint and muscle imbalances are identified. You are learning how to properly perform essential corrective movements in an effort to reduce these symptoms and restore movement.

3

Improve Your Strength, Function & Endurance

At this point you have nearly full, pain-free motion, improved motor control and are able to perform specific types of movements properly yet still require some level of supervision.

The overall goal of PHASE 3 is to improve functional capacity, activity tolerance and endurance. During Phase 3 you will be monitored and progressed by your treatment team to ensure you maintain your optimal level of movement. Treatment teams will often perform functional testing during this phase to measure both success and outcomes. Normal work, sports or other forms of physical activity are resumed.

4

Transition Into Your Personal Independent Home or Fitness Program

PHASE 4, or the Wellness Phase, marks the point in the journey where the you’re able to resume your normal daily activities. You will be presented with the option of beginning a total body fitness program, if you choose to progress to this level of activity. Remaining or becoming more physically active will help ensure an improved quality of health and life.
